When Is Gallbladder Surgery Needed?

Cholecystectomy is recommended for:

• Symptomatic gallstones — recurrent biliary colic (right upper abdominal pain after fatty meals)
• Acute cholecystitis — infected, inflamed gallbladder (emergency surgery within 72 hours preferred)
• Gallstone pancreatitis — stones blocking the common bile duct
• Gallbladder polyps >10mm — risk of malignancy

Asymptomatic gallstones generally do not require surgery unless the patient has a porcelain gallbladder or very large stones (>3cm).

Laparoscopic Cholecystectomy

The standard approach uses 4 small incisions (5–12mm each). A camera and instruments are inserted to detach the gallbladder from the liver and extract it through the umbilical port. The procedure takes 30–60 minutes.

Key safety steps:

• Critical View of Safety (CVS) — the surgeon must clearly identify the cystic duct and cystic artery before clipping to avoid bile duct injury (the most serious complication, occurring in 0.1–0.3% of cases)
• Intraoperative cholangiogram — X-ray of the bile ducts during surgery to confirm anatomy (recommended but not universal)

Single-Incision (SILS) Technique

Some surgeons offer single-incision laparoscopic surgery (SILS), using one cut hidden within the belly button. Cosmetically superior, with virtually no visible scar. Available at most advanced centres in India and Thailand.

Recovery and Diet

• Day 1: most patients discharge same day or next morning
• Week 1: mild shoulder pain (from CO₂ gas), minimal wound discomfort
• Week 2: return to desk work; avoid heavy lifting
• Week 4: full activities including exercise
• Diet: low-fat diet for 2–4 weeks; gradual reintroduction of fats. Most patients eat normally by 3 months
